What is a Ketamine Assisted Psychotherapy (KAP)?

KAP is a integrated approach supports a more holistic ketamine treatment with focuses psychotherapy sessions to deepen the therapeutic, leading to more frequent insights and improve clinical outcomes for effective healing process. 

In a 2019 study led by Jennifer Dore, patients experienced significantly decreased depression &amp; anxiety scores (HAM- A & BDI at p<0.0001) in private practice setting with a licensed mental health professional vs. receiving ketamine alone. At Atomic Mindworks we have built a platform around the therapeutic alliance. In our
patient-centered collaborative care model. This approach allows patients to remain in therapy with existing clinicians, receive personalized treatment plans, and experience comprehensive care.

What is Ketamine?

Ketamine is a dissociative anesthetic, commonly used in a surgery and pain management. In recent years, it has also been prescribed as a fast-acting antidepressant and  as a psychedelic adjunct to psychotherapy. Approved by FDA for mental health treatment since 2019, it is considered safe when administered under medical supervision. Ketamine can be delivered through various methods, including intravenous infusion, intramuscular injection, nasal spray, and sublingual lozenges. At Atomic Mindworks, we utilize sublingual ketamine in conjunction with psychotherapy, known as Ketamine-Assisted Psychotherapy (KAP), to enhance accessibility and cost effective for patient.

Ketamine works by blocking NMDA receptors and stimulating AMPA receptors, promoting the formation of new synaptic connections and enhancing neuroplasticity, which can lead to lasting symptom improvement.

 

During KAP sessions, patients may experience a sense of detachment and shifts in perception, often gaining valuable insights. Overall, ketamine's ability to reduce negative thought patterns and build new neural connections offers a transformative potential for individuals seeking mental health support.

Safety Profile
Ketamine has been FDA approved for anesthesia and analgesia since 1970, and as a mental health treatment since 2019, and is currently listed on the World Health Organization's essential medications list. Its track record of safety in emergency rooms, outpatient and psychiatric settings has created an extensive body of clinical evidence that supports its safety for psychotherapeutic use (Gao et al., 2016). The side effects of ketamine are mild and transient (if they do occur) and it can be safely administered with most other medications. ’s patient-centered collaborative care model has rigorous patient safety policies and procedures in place to safeguard patient health and support improved clinical outcomes.

Routes of Administration

There are several ways to work with ketamine, including IV infusion, intramuscular injection, and lozenges. This method is more accessible and cost-effective for patients. At Atomic Mindworks  we  may prescribe ketamine in the context of Ketamine-Assisted Psychotherapy (KAP), in which a patient is in a therapeutic alliance with a licensed KAP Facilitator.

Ketamine and the Brain

Ketamine has rapidly acting antidepressant and mood-enhancing effects, which can begin to take effect within 1-2 hrs of administration. It works by blocking the brain’s NMDA receptors as well as by stimulating AMPA receptors, which are thought to help form new synaptic connections and boost neural circuits that regulate stress and mood. Ketamine has also been shown to enhance overall neuroplasticity for lasting symptom improvement.

How Ketamine Feels

The effects of ketamine last for approximately 45 minutes. These effects can make patients feel "far from" their body and facilitates shifts in perception that can often feel expansive in nature. Motor and verbal abilities will typically be reduced. During Ketamine-Assisted Psychotherapy, patients typically lie down in a comfortable position during their dosing session, wearing headphones and an eye-mask. While it may feel hard to articulate what happens during the experience, many patients feel like the insights gained are clear.

The United States is experiencing an ever-increasing mental health crisis, which has only been exacerbated by the socially isolating COVID-19 pandemic. In 2023, 45% of adults ages 35 to 44 reported a mental illness, compared to 31% in 2019 (apa.org). People living with mental illnesses, such as chronic anxiety and depression,
tend to have less flexible neurons with fewer connections, amongst other structural issues.  Ketamine can provide safe and rapid antidepressant relief, revitalizing the brain’s ability to form new connections and enhancing overall receptiveness to psychotherapy.


Ketamine’s therapeutic effects stem from a variety of mechanisms in the brain:

  •  Increased BDNF: Ketamine increases Brain-Derived Neurotrophic Factor (BDNF), which has complex downstream effects. This effect is dose-dependent and time-limited (around one week, with peak at 2-3 days).

  • Lowering the Default Mode Network: Ketamine has been shown to reduce the functional activity of the Default Mode Network (DMN), which can give the patient a chance to take a break from habitual thoughts and gain perspective.

  • Enhanced Neuroplasticity: Ketamine enhances neuroplasticity by upregulating BDNF. This effect opens a window of opportunity for psychotherapeutic intervention.

  • Intrinsic Anti-Depressant Effects: Ketamine has rapidly acting antidepressant and mood-enhancing effects, with a response rate of approximately 50-80% and duration ranging from 2-3 days up to 2 weeks or more. Evidence also supports a unique anti-suicidal effect, independent of antidepressant response.

  • Dissociation: Ketamine's dissociative effect may provide patients the opportunity to gain a new perspective on a traumatic or unpleasant situation.

  • Responsiveness to therapy: Following a Ketamine dosing session there is a  critical period which allows for higher responsiveness to therapy, caused by the effects.

Since the start of the Covid-19 pandemic, the incidence of mental health diagnoses has doubled, and today, nearly half of the US population suffers from some form of depression or anxiety condition. We urgently need effective, novel tools to help ease the burden of those suffering and improve their long-term outcomes.
Of all the psychedelic medicines, the most studied has been ketamine. Over the last several decades, ketamine’s record of clinical safety in ER, outpatient and psychiatric settings has produced an extensive body of clinical evidence which supports its safety for use in psychotherapeutic settings (source: Dore et al, 2019). Leveraging ketamine as an adjunct to psychotherapy, has been particularly effective for significantly
improving clinical outcomes.  While in the short term, ketamine can provide rapid relief in symptoms through increased neuroplasticity, psychotherapy supports the process of meaning-making and the cultivation of anchors through new practices and ways of being that both primes the brain for these new insights and then supports their ongoing integration.

A 2019 study by Jennifer Dore et al. which examined outcomes from three psychotherapy practices using Ketamine-Assisted Psychotherapy for hundreds of patients over the course of several years concluded that KAP is “an effective method for decreasing depression and anxiety in a private practice setting, especially for…those with severe symptom burden” (Dore et al, 2019). In this study, participants with severe symptom burden (i.e. psychiatric hospitalization, suicidality at intake and within the past year) who underwent KAP treatment demonstrated higher levels of improvement in well-being, anxiety, Beck Depression Inventory, Hamilton Anxiety Scale, PTSD scores and drug and alcohol use scores. Further analysis of the improvement in
anxiety and depression demonstrated that people with PTSD or developmental trauma showed the highest level of improvement in depression and anxiety scores (Dore et al, 2019). Data from the study supports the efficacy of KAP for a host of psychiatric conditions, rapidly and significantly decreasing depression, anxiety and
PTSD.

Many trials have shown that ketamine and other psychedelics produce better clinical results when used in conjunction with psychotherapy: *MDMA is not currently an FDA approved psychedelic, someday soon we hope!


At Atomic Mineworks we practice a collaborative care model in which members (trained KAP facilitators) take on the therapy portion of the experience (including preparation, set and setting, pre session intention setting and integration sessions), while Rena Apon CRNA PMHNP supports patients in all medical aspects. Instructions For Use

How KAP Works with Atomic Mindworks

KAP at Atomic Mindworks involves two key components: medical care and psychotherapy. License counselor in our network handle the counseling aspect while our in-house medical team overseas the medical side.

Begins with an initial consultation with  a KAP-trained prescriber from with our PMHNP. Patient's medical and psychiatric will be reviewed during this evaluation to assess eligibility for KAP if eligible, a personalized treatment plan is created.

Each Atomic Mindworks patient will be guided through their KAP journey once the patient has their ketamine prescription, the facilitator will plan the following session:

Preparation Sessions

These sessions help the patient and facilitator establish goal and expectations. They take place before the first ketamine dosing session and are tailored to the patient's treatment plan.

KAP Dosing Sessions

In these 1-3 hour sessions, patients self-administer a ketamine troches, typically while seated comfortably with calming music. The facilitator is present throughout to offer support as needed. Patients usually have 6-8 dosing sessions.

Integrating Sessions

After each dosing session, the patient and facilitator meet to reflect on insights gained and prepare for future sessions.

Step 3: Consultation and Ongoing Monitoring

It’s essential to have regular consultations with us while using ketamine troches. This ensures the effectiveness of the treatment and allows for any necessary adjustments.
